Locate your child's school by visiting the school map page.  

For information about kindergarten roundup, please contact your home school for specific dates and times.

Visit the Davis School District Home page and select the school from the drop-down menu at the top of the page. Some schools have registration packets available on their school website. During the school’s office hours listed on their website, arrive in person and provide the following required information: 
o Parent picture identification

o Original birth certificate for the student
o Original student immunization schedule
o Proof of address with a current rent/lease or purchase agreement AND one of the the following: bank statement, utility bill, government letter, postal change of address. 

Immunizations required by the state of Utah must be completed and documentation brought to the school for a student to be registered. See the Utah Department of Health Immunization Program for a list of immunization requirements.
